| Are you filing one of the following forms? Petition for Individual Studies, Part Time Final Quarter Approval, Request for Drop of Major/Minor, Veterans Enrollment Verification, or UCSC Extension Concurrent Enrollment |
Scan and email your form to your advisor or drop your form off at our offic 142 JBEB (or use our drop-box located just outside our main office door). Your advisor will review your form and send a completed scanned copy to your UCSC email address.
Do you need to declare your Biological Sciences major?
To declare a single major, or change from a single major to a new single major use the online Declaration of Major Petition available here:

| Do you need to see an advisor? |
| There are two ways to meet with an advisor: by appointment or at drop-in. |
| Use our online appointment system to schedule a 20 minute one on one appointment with your program advisor. |
| Appointments can be made up to two weeks in advance. Appointment times more than two weeks out will show in the appointment system as unavailable. |
| For drop-in advising see the schedule below. Drop-in is on a first-come/first-serve basis. Sign in for drop-in advising in 142 JBEB; the sign-up sheets are available 30 minutes prior to the start of each drop-in session. |
